- Letter/ Evidence of Local Government of Origin (Original and 5 copies)
- Reference Letter (Original and 5 copies)
- School Charges Receipt (Original and 5 copies)
- Birth Certificate/ Declaration of Age (Original and 5 copies)
- Bio Data Form (5 copies)
- Course Form (5 copies)
